Why Your Lips Look Uneven in the First Week After Filler

The first week after lip filler is when most people panic. One side looks fuller. The cupid's bow looks off. The border looks uneven. Photos from day two feel nothing like the result you were expecting.

In most cases, that is not a failed treatment. It is swelling, product settling and normal healing. Understanding that timeline stops a lot of unnecessary worry, and it helps you know when a review appointment is actually useful.

What usually happens in the first seven days

Lips hold fluid easily. After filler, that fluid rarely sits in a perfect mirror image on both sides. Sleeping on one side, talking, eating and even smiling can make one area look temporarily higher or rounder.

A typical pattern looks like this:

  • Day 0 to 1: Immediate volume plus early swelling. Symmetry can look worse before it looks better.

  • Day 2 to 3: Peak puffiness for many people. Bruising may appear. This is when unevenness feels most dramatic.

  • Day 4 to 7: Swelling starts to calm. Shape becomes easier to judge, though it is still not the final result.

  • Week 2 onwards: Product integrates more fully. A review makes more sense if something still feels off.

If you are comparing selfie angles taken hours apart, you will almost always find a difference. That difference is often fluid, not filler placement gone wrong.

Uneven does not automatically mean the filler is wrong

True asymmetry from placement can happen, but it is less common than temporary swelling asymmetry. Your lips were also unlikely to be perfectly even before treatment. Filler can make a small natural difference more noticeable while everything is inflamed.

A clinician will usually wait for the early swelling phase to settle before deciding whether a small tweak, massage advice, or simply more time is the right next step. Dissolving filler in week one is rarely the first conversation unless there is a medical concern.

What helps in the first week

Follow the aftercare you were given. In practical terms, that usually means:

  • Avoid pressing, kneading or "evening out" the lips yourself

  • Skip intense heat, saunas and strenuous exercise for the advised window

  • Be gentle with makeup around the injection points

  • Sleep slightly elevated if your clinician suggested it

  • Contact the clinic promptly for sudden severe pain, spreading redness, blanching or vision changes

Home remedies that claim to force filler into place are not a substitute for clinical review. If something feels wrong, message the clinic rather than trying to fix it in the bathroom mirror.

When to book a review rather than wait

Wait-and-see is sensible for mild day-to-day unevenness. Book sooner if you have marked pain, skin colour changes, a cold-sore style outbreak, or swelling that is getting worse rather than slowly settling.

For shape concerns that are purely aesthetic, a review around two weeks is often more useful than one at 48 hours. By then, the clinician can see the real contour rather than temporary fluid.

Choosing lip filler with a clear recovery plan

A good lip consultation covers more than volume. It should include how much product is realistic for your lip anatomy, whether a staged approach is wiser than doing everything in one sitting, and what your first week is likely to look like.

At Beautoxology, lip filler is bookable through Phorest. Clinics are in Belbroughton, Stow-on-the-Wold, Cirencester and Much Wenlock, with patients travelling from Birmingham, Edgbaston, Halesowen, Hagley, Bromsgrove, Evesham and across the West Midlands and Cotswolds.

If you are planning treatment and want a natural result with a clear aftercare plan, book a consultation. If you are already in your first week and unsure what you are seeing, get in touch rather than guessing from photos alone.
