Lip Flip or Lip Filler: How to Choose

You want your upper lip to look a little fuller in photos, but you do not want obvious filler. Maybe the lip disappears when you smile, or the top lip looks thin next to a stronger lower lip.


That is usually when people land on the same question: lip flip or lip filler? They sound similar online. In clinic they do very different jobs. Picking the wrong one is how subtle requests turn into too much volume, or why a flip feels disappointing when you actually needed shape work.


What a lip flip actually does

A lip flip uses Botox in specific points around the upper lip to soften muscle pull that can tuck the lip inward. The lip looks slightly more visible at rest, especially along the pink border, without adding volume inside the lip.


It is a small, temporary change. Think “a little more show” rather than “more size.” Results typically last a few months, similar to other Botox treatments in mobile areas.


What lip filler actually does

Lip filler adds structure and volume with hyaluronic acid gel. It can refine the border, balance asymmetry, restore lost fullness, or build shape in the cupid's bow and body of the lip. That is a different outcome from a flip, even when the amount used is conservative.

Choose a lip flip if…

  • Your main issue is the upper lip curling under or looking thin when you smile

  • You like your lip size but want a bit more visibility at rest

  • You want the lightest possible intervention first

  • You are nervous about volume and want to avoid the “overfilled” look entirely


Choose lip filler if…

  • You want actual volume, better symmetry, or clearer shape definition

  • Fine lines on the lip surface or a flat border are part of the problem

  • The lower lip is fuller and the upper lip needs balance, not just a slight roll

  • You have had a flip before and liked the direction, but wanted more change than Botox alone can give

Can you combine them?

Sometimes, yes. A flip can soften a gummy or tucked appearance while a small amount of filler addresses border definition or symmetry. That combination should be deliberate, not the default. More products is not automatically more natural.


A staged plan often works best: start with the treatment that targets the main complaint, review at two to four weeks, then decide if anything else is genuinely needed.


When neither may be the answer

If the concern is mainly dry skin, dehydration lines or lipstick bleeding, skincare and lip care may come first. If the issue is tooth show or gum display when smiling, the conversation may point toward a gummy smile Botox assessment rather than filler. If previous filler is still present and distorting shape, dissolving or adjusting old product may need to happen before anything new is added.
