The First 48 Hours After Botox: What Is Normal and What Is Not

You are home after your appointment. The injection itself is usually quick, typically around 10 to 20 minutes at the clinic. If this was your first visit, you will also have had a full consultation beforehand to assess suitability and discuss your goals. Now you look in the mirror and wonder whether what you are feeling is normal.

It is a fair question. Botox takes a few days to work, and the first 48 hours can feel uncertain if nobody has explained what to expect. This guide covers the usual experience, sensible aftercare, and the rare signs that mean you should pick up the phone.

What you will not see in the first 48 hours

Botox does not work instantly. The muscle relaxant needs time to bind and take effect. In the first day or two, your face will look much the same as before treatment. Some patients worry nothing has happened. Others convince themselves they already see a change. Neither is reliable. Most people notice softening between day three and day seven, with full results around two weeks.

What is normal

Mild redness or small bumps at injection sites

These usually settle within an hour or two. A tiny pinprick mark may linger slightly longer. This is expected and not a sign of anything going wrong.

Tenderness or a slight headache

A mild headache on the day of treatment happens occasionally. Simple paracetamol is fine if you need it. Avoid ibuprofen and aspirin unless you were already taking them on medical advice, as they can increase bruising risk.

A faint bruise

Not everyone bruises, but it is possible, especially around the eyes or if you are prone to bruising generally. A small mark can take a few days to fade. Your clinician should have discussed this at consultation.

Feeling aware of the treated area

Some people describe a slight heaviness in the forehead or tightness when raising the brows. This typically eases as the product settles. It does not mean the result will look frozen.

Aftercare for the first 48 hours

Your clinic should give you specific instructions. In general:

Stay upright for four hours after treatment. Avoid lying flat or bending over repeatedly.

Do not rub or massage the treated areas. This includes facials, vigorous skincare application and wearing tight headwear that presses on the forehead.

Skip strenuous exercise, saunas and hot yoga for 24 hours. Heat and increased blood flow are best avoided on the day.

Hold off on alcohol for 24 hours if you can. It will not ruin your result, but it can worsen bruising.

You can wear makeup gently the next day if there is no broken skin. Light application, no aggressive blending over injection points.

What is not normal

Contact your clinic promptly if you experience:

Drooping of the eyelid or eyebrow that develops or worsens after day one.

Difficulty swallowing, speaking or breathing.

Vision changes or significant pain beyond mild tenderness.

Signs of infection: spreading redness, warmth, pus or fever at an injection site.

These are uncommon when Botox is administered by a registered medical professional, but they matter. A reputable clinic will want to hear from you and will have a plan.

Planning around work and social events

If you have an important meeting, wedding or photograph coming up, book at least two weeks ahead. That gives the result time to settle and any minor bruising time to fade. Patients from Evesham and the Vale often schedule mid-week appointments and are back at work the same day, but it is worth building in margin for a first treatment when you do not yet know how your skin will respond.

When to book your next appointment

Results typically last three to four months, sometimes longer with regular treatment. Your clinician can advise on timing at a follow-up or review. There is no rush to rebook on the day of treatment.
